National Healthcare Decisions Day
April 16, 2012
Share Your Wishes!

The Idaho End-of-Life Coalition is promoting a grassroots campaign to highlight the importance of advance healthcare decision-making.  The kickoff coincides with the April 16, National Healthcare Decisions Day (NHDD), which has been recognized for five years by the National Hospice and Palliative Care Organization (NHPCO). Talking about end-of-life wishes is an important conversation to have, but is often put off until it is too late.  As an incentive to have these conversations and complete advance directives, the Idaho End-of-Life Coalition is promoting the first annual “Choices Raffle with a prize trip to LAS VEGAS!

How To Enter?

1. Complete your advance directive- forms available on registry website
http://www.sos.idaho.gov/general/hcdr.htm

2. Complete registry form- available at website mentioned in step 1

3. Mail directive and form to:
Idaho Secretary of State
Attn: Health Care Directive Registrar
700 West Jefferson Street, Room E205
PO Box 83720
Boise ID 83720-0080

Prize includes:
• Airfare
• Transport to and from airport
• Lodging at a resort on the Las Vegas strip!

The Choices Raffle is an example of the

IdahoEnd-of-Life Coalition’s goal to empower the public to make informed choices related to end-of-life care. According to a statewide survey, 98% of Idahoans have heard of living wills for end-of-life healthcare preferences, but only 35% have completed one (2006, Center For the Study of Aging, Boise State University and Idaho End-of-Life Coalition).

National Healthcare Decisions Day exists to inspire, educate & empower the public & providers about the importance of advance care planning. National Healthcare Decisions Day is an initiative to encourage patients to express their wishes regarding healthcare and for providers and facilities to respect those wishes, whatever they may be.

2012 IDAHO
LEGISLATIVE UPDATE

HB 1294 has passed without opposition! This is the update we all have been working on for the Idaho POST. Your coalition's Policy Committee worked for the past year to make these changes to improve healthcare and to ensure that patient wishes are known and followed. The most important update to the Idaho POST is that now Physician Assistants and Nurse Practitioners can now sign the POST.
